We hope that the information provided on this website can help you to live a healthy, safe and independent life for as long as possible. We’ve included lots of information and advice about services that you can access now to support your independence, including:
Our self-help tool
You can complete our self-help tool. This will:
- look at your needs and abilities
- give you information on local services which can help you
- tell you if you may benefit from completing an online self-assessment.
If you have explored all the options available to you but still feel that you need more support then you can contact us to discuss your needs. You can also ask us to carry out a care needs assessment.
Talk to us
If you would like to talk to someone please contact Care Direct on 0345 1551 007 or email firstname.lastname@example.org.
When we first talk to you we will listen carefully and won’t assume anything. We want to find out:
- What matters to you today?
- What you can do for yourself?
- What are your interests and skills?
- Do you have family and friends locally?
- What are your concerns and what do you want to achieve?
We will ask about the skills and resources you have and the access you have to support from friends, family and the wider community. Using this information we will work with you to try and resolve your needs and achieve a desired outcome. We will want to help you to use your strengths in order to remain as independent as possible for as long as possible.
If you just need a little bit of help, for example, to remain living safely at home then we can provide advice about where to buy equipment or how you could go about making some adaptations. You might just want to employ a cleaner. Pinpoint Devon is the place to start.
If you have more complex needs then you might need a more detailed discussion about the help you need. This could happen over the phone or face to face.
You may also be offered an assessment of your care and support needs and your eligibility for support
