If you receive travel assistance as part of your SEND assessment, this will be reviewed on an annual basis and the transport team will contact you. 

Support to help you travel to school will end when you are 19. 

You may be eligible for support from adult services as part of your care package – we will talk to you about this on an individual basis dependent on your plans.
