What is meant by ‘transition’?


Simply put, a transition is a time of change. You may go through lots of different transitions, and we may offer to work with you at different times to make sure you have the right support you need at the right time.

Here are some examples of transitions that we will be discussing in this route map. Transition from:

  • children’s social care to adult social care – or independence from social care
  • school to college, higher education or employment, training, or apprenticeship
  • residential or foster care placements to semi-independent or independent living
  • college, higher education, vocational training, or apprenticeships to employment or adult social care (or both) or health
  • children’s to adult health services
  • being a child in care
  • living at home to semi-independent or independent living

Our support for transitioning to adulthood usually starts when you are in academic year 9 (aged 13 to 14). We’ll let you know how to get in touch with us or you fill out a referral form if you would like our involvements. Details will be provided later in this guide.

Either you, your parents or carers or someone else involved in your life can do this as long as they have consent from you or your parents or carers. This includes any professionals currently working with you or your family.
