What we will need from you
When providing evidence for an EHC needs assessment, you should consider including the following:
An account of your child’s needs: You can write a detailed account of your child’s difficulties, the impact on your daily life and education, and any support you believe your child needs.
Reports from your child’s educational setting: This could include observations, assessments, and any interventions that have been tried within the school or early years setting.
Doctors’ assessments of your child: If your child has seen any medical professionals for their needs, any resulting reports or assessments can be very helpful.
Evidence of your child’s physical, emotional, and social development: This can include reports from clinicians and other health professionals, as well as information on what has been done to meet these needs by other agencies.
Information on your child’s progress: Evidence that, despite relevant and purposeful action taken by the early years provider, school, or post-16 institution to identify, assess, and meet the special educational needs of your child, they have not made expected progress.
It’s important to provide a comprehensive picture of your child’s needs and the support they require. We will use this evidence along with all the other advice gathered to determine whether an EHC plan should be made for your child.

Submitting your evidence on the EHC Hub
There are two areas for you to record your views as well as those of your child. ‘About Me’ and ‘Parent/Carer Views’ are multimedia spaces where you can type text, add photographs or short video clips. If we decide to issue an EHC plan, we will use this information to form Section A of the plan.
