There are new Guidance for Transfer Reviews and Transfer Review Forms available from September 2105. Please make sure you are using the most up-to-date one.

There is a new EHC plan Annual Review Form for children with EHC plans. Please make sure you are using the right one. Children with statements who are not transferring should continue to be reviewed using the existing Annual Review Form.
