For current members, you will need to complete the ill-health certificates IHCURR1 and IHCURR2.
Both certificates are also available in Employer Self-Service and can be added as extra pages to the leaver form.
All the certificates listed below are not available in Employer Self-Service so once completed, please send them to our secure mailbox pensionsbenefitssecure-mailbox@devon.gov.uk.
Ill-health reviews
- Third tier ill-health retirement (IH3RW1A) – 18 months’ review (ceased employment before 1 April 2014)
- Third tier ill-health retirement (IH3RW1B) – 18 months’ review (ceased employment after 31 March 2014)
- Third tier ill-health retirement (IH3RW2) –employer’s review decision
Suspended tier 3 ill-health pensions
- Review whilst pension in payment or within 3 years after payments stopped (IH3SUS1) (ceased employment after 31 March 2014)
- Review taking place 3 or more years after payments stopped (IH3SUS2) (ceased employment after 31 March 2014)
- Review taking place within 3 years after payments stopped (IH3SUS3) (ceased employment before 1 April 2014)
- Review taking place 3 or more years after payments stopped (IH3SUS4) (ceased employment before 1 April 2014)
Early payment of deferred benefit on ill health grounds
- Employee left on or after 1 April 2014 (IHDB1D)
- Employee left on or after 1 April 2008 and before 1st April 2014 (IHDB1A)
- Employee left on or after 1 April 1998 and before 1st April 2008 (IHDB1B)
- Employee left before 1 April 1998 (IHDB1C)
- Ill-health employer decision – early payment of deferred benefit (IHDB2)
All ill-health certificates need to be signed by an approved independent registered medical practitioner (IRMP) and our team will check the name of the doctor against our approved list before processing any retirement benefits. The approved list is kept internally but you can request a copy from the Employer and Communications Team via the email link below.
You should have already nominated which Doctor or Company you are using but if you haven’t informed us or are changing who you use, please email finance.peninsulaemployers-mailbox@devon.gov.uk with their:
- full name
- General Medical Council (GMC) Number
- practice/contact details
- email address
- telephone number
Along with a copy of the OCH qualification certificate and we will then add them to our approved list.
